The Project Manager is responsible for planning, coordinating, and executing organizational projects that support Axiom Care's strategic, operational, and clinical objectives. This position serves as the central point of coordination among leadership, clinical teams, business operations, community partners, and external stakeholders to ensure projects are completed on time, within scope, and within budget.
The Project Manager develops project plans, tracks progress, manages risks, facilitates communication, and ensures accountability across departments. This role supports organizational growth by driving operational efficiency, process improvement, and successful implementation of strategic initiatives.

Duties/Responsibilities
· Leads the planning, implementation, and oversight of assigned organizational projects from initiation through completion.
· Develops detailed project plans, timelines, milestones, deliverables, and resource requirements.
· Monitors project performance, identifies risks, and implements corrective actions to maintain project timelines and objectives.
· Facilitates project meetings, prepares agendas, and documents action items and project updates.
· Maintains project documentation, status reports, dashboards, and performance metrics.
· Collaborates with leadership to define project goals, scope, budgets, and success measures.
· Tracks project expenditures and assists with budget management and resource allocation
· Ensures compliance with organizational policies, contractual obligations, regulatory requirements, and grant deliverables
· Coordinates communication among internal departments, external partners, government agencies, tribal organizations, and community stakeholders as applicable.
· Supports implementation of technology, behavioral health, healthcare, operational, and infrastructure initiatives.
· Identifies process improvement opportunities and recommends operational efficiencies.
· Assists with data collection, reporting, performance measurement, and outcome tracking.
· Provides regular progress reports for executive leadership and key stakeholders.
· Supports change management efforts associated with new programs, services, systems, and operational initiatives.
· Maintains project management tools and ensures project records are accurate and up to date.
· Coordinates vendor activities, contract deliverables, and external consultant engagements when assigned.
· Performs other duties as assigned.

Education and Experience:
· Bachelor’s degree in business administration, Healthcare Administration, Public Health, Project Management, Organizational Leadership, or related field required.
· Minimum of three (3) years of project management, operations, healthcare, behavioral health, or related experience.
· Experience coordinating cross-functional teams and managing multiple concurrent projects preferred.
· Experience in healthcare, behavioral health, tribal health, human services, or community-based organizations preferred.
· Project Management Professional (PMP), CAPM, Lean Six Sigma, or equivalent certification preferred.
Physical Requirements:
· Prolonged periods of sitting and working on a computer.
· Ability to occasionally lift up to 10 pounds
· Ability to travel locally and regionally as needed for meetings, project implementation, and stakeholder engagement.
